Thank you for reviewing the LAAO Registry Monthly Case Scenario for January of 2020. We will be discussing sequence number 13060 in version 1.3 and sequence 14882 in version 1.4, follow-up atrial thrombus detected. We have documentation of a 56-year-old male patient who underwent a successful LAAO procedure in which a Watchman device was implanted. Documentation from the clinician's office in which the 45-day follow-up assessment took place is being used for abstraction. The patient had their follow-up TEE a day prior to the office assessment. The follow-up assessment documentation notes the TEE findings as the patient having a 0.5 millimeter leak and a device thrombus detected on the atrial face of the device. Based on the follow-up assessment documentation, how will sequence 13060 or 14882 in version 1.4 atrial thrombus detected be coded? Number one, no, or number two, yes. Please take a few moments to review the documentation and the question to determine the best answer. Based on the documentation, sequence 13060 or sequence 14882 atrial thrombus detected will be coded as yes. Based on the information given, the thrombus was documented to be located on the atrial side of the Watchman device, not in the left atrial appendage side that has been blocked by the Watchman occlusion device. Therefore, please code yes to atrial thrombus detected in sequence 13060 or 14882 and device thrombus as a follow-up event. If the thrombus detected is located inside the left atrial appendage, in other words, behind the newly placed device, please code no to sequence 13060 or 14882 atrial thrombus detected as the thrombus is now sealed off from the left atrium. Thank you for viewing the LAAO Registry Case Scenario for January of 2020.
Video Summary
In this video, the LAAO Registry is reviewing a case scenario for January 2020. The scenario discusses a 56-year-old male patient who underwent a successful LAAO procedure with a Watchman device implant. The patient had a 45-day follow-up assessment, during which a transesophageal echocardiogram (TEE) revealed a 0.5 millimeter leak and a thrombus on the atrial face of the device. The viewer is asked to code this as either yes or no for sequence numbers 13060 or 14882 in version 1.4. Based on the documentation, the correct coding would be yes for atrial thrombus detected, as it was found on the atrial side of the device. If the thrombus had been located inside the left atrial appendage, behind the device, the coding would be no.
