Let's review the executive summary measures and metrics companion guide for version 2.3. There was a recent update to metric 34. The numerator includes those patients with a primary tachycardia indication present that is coded in sequence 14729 and a bradycardia indication present that is coded in sequence 14730. The denominator includes all patients with sequence 7620 device implanted coded as yes and final device type coded as ICD dual chamber in sequence number 7625. This measure assesses the hospital's rate of dual chamber ICD implants without a bradycardia indication. The question to ask when evaluating the data is, what was the purpose of implanting the RA lead? Does the patient have symptomatic sinus node dysfunction, sick sinus syndrome, sinus bradycardia, or tachybrady syndrome documented in the medical record? If any of these arrhythmias are symptomatic, they are coded in sequence 14731. Reason pacing indicated as sick sinus syndrome. Does the patient have complete heart block, chronotropic incompetence, Mobitz type 2, or a 2 to 1 AV block documented in the medical record? If so, code sequence 14731 reason pacing indicated as documented. If the RA lead is implanted for SVT discrimination, sequence 14731 reason pacing indicated is left blank. When the reason for implanting an RA lead is not evident in the medical record, please consult with the implanting clinician.
