The case scenario will be focused on the capture of attempt at atrial fibrillation termination, sequence 4410 and 4420. Our patient, Mrs. Walker, was admitted for a catheter-based AFib ablation, for which the plan strategy was cryoablation. Her past medical history includes long-standing persistent AFib, an ICD implant, CHF with LV dysfunction and an EF of 35%, hypertension, and electrophysiologist documentation of ICD delivering shocks for AFib and at times successfully terminating the rhythm. When capturing Mrs. Walker's rhythm history, will the documented ICD shocks be captured as attempts to terminate AFib in sequence 4410? Number one, no, or number two, yes. Please review the documentation and questions to determine the best answer. We will give you a few seconds to review. And the answer is number two, yes. Even if the ICD shock for an AFib rhythm is documented as an inappropriate shock, a direct current shock was administered to the patient to attempt to terminate the AFib rhythm. Therefore, we would capture the ICD shocks as an attempt to terminate AFib in sequence 4410 and code as direct current cardioversion in sequence 4420.
