This month, we will be discussing the coding of sequence number 4040, Chad's VASC, thromboembolic vent. In this scenario, our patient, Mr. Jones, is admitted for an ablation procedure. Mr. Jones has a documented history that includes paroxysmal AFib, hypertension, CHF, and a prior MI with a PCI. There's also a past history of DVT. The provider did not document their own Chad's VASC assessment of Mr. Jones. Here's our question. Based on the documented history of DVT, how will sequence number 4040, Chad's VASC, thromboembolic event, be coded? Number one, no. Number two, yes. Please take a moment to review the documentation in question before making your final answer. The answer is no.
