Hello, everyone, and welcome back to our bi-monthly case scenario for February 2025. For this case scenario for the LAAO registry, we will be discussing error FI-22509. We will be looking at the cause of the error as well as how to resolve it. Our documentation is as follows. Mr. P had a successful LAAO procedure on August 5th of 2024. Upon entering Mr. P's 45-day follow-up, the abstractor finds a phone call between the provider's office and the patient on September 23rd of 2024, in which medications were discussed. The abstractor enters this phone assessment as the patient's 45-day follow-up entry and is submitted. The assessment date is coded as September 23rd of 2024. While discussing Mr. P's 45-day follow-up with another abstractor, an office assessment with the implanting provider, which will take place on October 2nd of 2024, is found. The abstractor makes the decision to use this follow-up assessment as the 45-day follow-up as it is a more complete assessment of the patient. The abstractor enters into the data collection tool, accesses the patient's follow-up entry, and changes the assessment date from September 23rd to October 2nd, 2024. Upon submitting the updated follow-up with the new assessment date, the follow-up submission receives a red inclusion status. When we assess the cause of the failed submission, we see the error causing the failure is error 5I22509, follow-up interval uniqueness, which means there are multiple 45-day follow-up entries for this patient, Mr. P. Error 5I22509 is triggered when the system has more than one follow-up entry for a specific interval. In this case, the system counts more than one 45-day follow-up for Mr. P. The main cause of the error is the change of the original assessment date coded for a new assessment date that is in a different quarter. For example, in Mr. P's case, the original date coded was September 23rd of 2024, which is a 2024 Q3 date, and was changed to October 2nd of 2024, which is a 2024 Q4 date. In order to clear the error, we need to clear the original follow-up entry from the system and make the new date the only 45-day follow-up entry for Mr. P. To do that, we need to resubmit the follow-up quarter of the original date, which is 2024 Q3. This will allow the quarter to go through the DQR process without Mr. P's original follow-up entry, clearing it from 2024 Q3 follow-ups. After submitting 2024 Q3, the quarter with the original assessment date, and letting the quarter go through the DQR process, we can move ahead and submit the quarter with the new assessment date, which is 2024 Q4. The error should now be cleared, and the submission should go through with a green or yellow inclusion status. Some additional things to know. Please do not submit both quarters at the same time. Allow the quarter with the original date to go through the DQR process, then submit the quarter with the new date. If a follow-up quarter submitted fails based on the error FI22509, please do not submit any other follow-up quarters until the error or errors have been cleared. Submitting more follow-ups prior to clearing the errors may compound the problem and increase the effort needed to fix the follow-up submissions.
