Thank you for reviewing the AFib Ablation Registry Monthly Case Scenario for March 2020. We will be discussing Sequence 7155, Number of Veins Isolated. We have documentation of a patient who is admitted for an AFib Ablation Pulmonary Vein Isolation Redo Procedure. Based on clinician documentation, after assessing all four pulmonary veins, one vein still isolated from the previous procedure, and three veins are no longer isolated. The procedure goes on and three veins are re-isolated, utilizing pulmonary vein isolation as the ablating strategy. When coding the procedure information, our question is, how will Sequence 7155, Number of Veins Isolated be coded? Will it be coded as two, three, or four? Please review the documentation and question to determine the best answer. We will give you a few seconds to review. Our answer is number two. We would code veins isolated as three veins. Sequence 7155 will capture the number of veins isolated during the current procedure being abstracted. Based on clinician documentation, our question is, how will Sequence 7155, Number of Veins Isolated be coded? Will it be coded as two, three, or four? Please review the documentation and question to determine the best answer. Based on the documentation, during the assessment, it was found that one vein was still isolated from the last procedure. They then targeted and isolated the veins found to be reconnected. When coding the procedure, we will code the number of veins present as four, number of veins targeted as three, and number of veins isolated as three. Thank you for reviewing the AFib Ablation Registry Monthly Case Scenario for March 2020.
Video Summary

After assessing all four pulmonary veins, one vein from the previous procedure was still isolated, while three veins were no longer isolated. During the current procedure, three veins were re-isolated using pulmonary vein isolation as the ablating strategy. The question is how to code Sequence 7155, Number of Veins Isolated. The answer is to code it as three veins isolated based on clinician documentation. The video concludes by thanking the viewer for reviewing the case scenario.
