Thank you for viewing the Chest Pain MI Registry March Case Scenario for 2022, where we will focus on inclusion criteria. A 70-year-old female arrived at the emergency department on 9-13-21 at 2-0-5 p.m. for a mechanical fall with a diagnosis of a left hip fracture. Medical repair of her hip was performed on 9-14-21 at 9 a.m. Two days post-op on 9-16 at 2-45 p.m., she developed chest pain and the ECG reveals ST elevation in the anterior leads. The patient is diagnosed with a STEMI and transferred emergently on 9-16-21 at 4-0-5 p.m. for PCI. She is a direct admit to the cath lab at Facility No. 2 on 9-16-21 at 4-35 p.m. Our question pertains to Facility No. 1. Is this patient included in the Chest Pain MI Registry? Is it No. 1, No? Or No. 2, Yes? Please take a few moments to review the documentation and the question before selecting your response. And the answer is No. 2, Yes. This patient is included in the Chest Pain MI Registry at the first facility. Let's review the Executive Summary Measures and Metrics Companion Guide to determine why. Per the inclusion criteria, patients arriving for scheduled procedures, as in elective caths and surgeries, are not included unless an in-hospital STEMI occurs and the in-hospital STEMI occurs after orders for admission. In other words, the diagnostic ECG occurs after cardiac or non-cardiac admission orders. Additionally, the in-hospital STEMI requires only a diagnosis of STEMI to be included in the registry. Our second question pertains to Facility No. 2. Is this patient included in the Chest Pain MI Registry? No. 1, No? Or No. 2, Yes? Please take a few moments to review the documentation and the question before determining your response. The answer is No. 1, No. This patient is not included in the Chest Pain MI Registry at the second facility. Let's review the Executive Summary Metrics and Measures Companion Guide to determine why. This in-hospital STEMI patient was transferred in less than 24 hours from symptom onset. However, the inclusion criteria document does not make any provision for this. The patient must be transferred within 24 hours of arrival to the first facility. This patient was transferred 38 hours after arrival to the first facility. Therefore, she is not included in the registry at the second facility. Thank you for viewing the Chest Pain MI Registry's March of 2022 case scenario regarding inclusion criteria.
Video Summary
This video discusses a case scenario from the Chest Pain MI Registry, focusing on the inclusion criteria. A 70-year-old female patient with a left hip fracture underwent a medical repair procedure. Two days later, she developed chest pain and was diagnosed with a STEMI. The patient was transferred to Facility No. 2 for PCI treatment. The question of whether the patient is included in the Chest Pain MI Registry at Facility No. 1 is answered as 'No. 2, Yes' based on the inclusion criteria. However, the patient is not included in the registry at Facility No. 2 due to being transferred 38 hours after arrival instead of within 24 hours. The video concludes by referencing the Executive Summary Measures and Metrics Companion Guide for further clarification.
