The LAAO case scenario for May is focused on sequence 13050, 45-day follow-up transesophageal echo performed. This element number was updated to sequence number 14874 to reflect the version 1.4 upgrade. The LAA occlusion procedure was successfully performed on February 10, 2020. The 45-day follow-up is due March 25, 2020. All elective TEE procedures have been canceled due to COVID-19. The question is, will this put the facility out of compliance with the LAAO Registry if the 45-day TEE cannot be performed within the follow-up window? Number one, no, or number two, yes. Please take a few moments to review the documentation and question before determining the best answer. Assessments performed during follow-up are at the discretion of the clinician or facility protocol. You may use telephonic or telehealth approaches to obtain all possible 45-day assessment data, which will be coded as phone call in method to determine status. If video is included with the telepath visit, code the method to determine status as phone call and other. If diagnostic tests such as a TEE cannot be performed at time of follow-up, please code no to that specific data element. Once elective testing is resumed, diagnostic tests may be performed. If the testing is performed within the 45-day follow-up date range up to day 91, please add to the phone call made at the time of 45-day assessment. If the testing is performed outside of 45-day follow-up date range after day 91, please record at the time of the six-month follow-up. Any medication changes stemming from the TEE results will be captured at six-month follow-up. The follow-up status, sequence number 13015, should be coded as lost to follow-up only with a documented attempt to obtain follow-up by phone call, mail, or email which have failed. If no attempt was made, no record should be submitted. If a follow-up record is unable to be obtained, your facility will understand when performing your internal quality reviews that any changes in metric results were related to this challenging period and unforeseen barriers to in-person follow-up visits. Once missing information is obtained and diagnostic assessment is performed, the LAAO registry allows for submission or resubmission of prior quarters of data at any time. Data will be re-aggregated, reflecting the updated information. Please remember, metric data is for optimizing patient care, your facility's use only and not publicly reported.
