In this scenario, we have a 56-year-old female with no significant past medical history that presents to the ED on May 5, 2020 at 7 a.m. with a chief complaint of acute onset shortness of breath and chest pain. She has a positive point-of-care troponin with an ECG that is positive for STEMI. The cath lab is emergently activated. The patient arrived in the cath lab at 745. Right femoral access was obtained at 757. After anticoagulants were administered, the interventional guidewire was inserted at 8 a.m. A drug-eluting stent was inserted at 812. However, it was unable to cross the lesion due to the tortuosity of the artery. After multiple attempts with multiple devices, the PCI procedure was aborted.
