Welcome to the STS-ACC TBT Registry May Case Scenario for Version 3. This month we will cover Section F, Follow-Up. A 74-year-old male status post-TAB or procedure in 2020 had a one-year follow-up assessment timeframe of 305 to 425 days post-procedure, which should have occurred between November 19, 2020 through March 19, 2021. Upon review of the medical record, it was noted a phone call with the daughter of the patient occurred in October 2020. During this conversation, it was learned the patient was in hospice with about one to two weeks to live. No other attempts to contact the patient or family were documented. Would a follow-up record be added during the November 19, 2020 through March 19, 2021 one-year timeframe? No or Yes. Please take a moment to review the documentation and question prior to making a final decision. And the answer is number one, No. For a follow-up record to be added during the one-year follow-up timeframe, an attempt to contact the patient or family must be documented to have occurred. Per the TAVR National Coverage Determination, Part A informs TAVR is covered for the treatment of symptomatic aortic valve stenosis when furnished according to a Food and Drug Administration approved indication and when all the following conditions are met. Specifically, No. 5, where the heart team and hospital follows the patient for at least one year. But since there was documentation of a phone call with the daughter in October 2020, can a follow-up record be added using the date of the phone call? And the answer is yes. A follow-up record that was documented to occur can be added to the data collection tool. However, without at least one follow-up assessment performed during the one-year follow-up timeframe, the information will not be harvested for inclusion in the dashboard metrics.
