The LAAO case scenario for May of 2021 focuses on sequence 13165, 45-day follow-up event systemic thromboembolism. This data element number has been updated to sequence 14948 to reflect the new updated version 1.4. A 75-year-old female with a history of TIA and hypertension had a successful LAAO procedure. Two months post-procedure, the patient presented to the emergency department with a two-day history of left calf tenderness while walking, redness in warm area at mid-calf, and shortness of breath. The PAO2 on room air was 92%. An ultrasound of the lower extremities showed a DVT and a CTA showed a pulmonary embolus and a heparin drip was started. The patient was admitted and the heparin drip continued for 36 hours with resolution of symptoms. Our question is, would the DVT and pulmonary embolism be coded as a follow-up systemic thromboembolism event? Number one, no. Or number two, yes. Please take a few moments to review the documentation in question to determine the best answer. And the answer is no. For the purpose of the registry, a deep vein thrombosis and or pulmonary embolus are not coded as a systemic thromboembolism. The upgraded version 1.3 added deep vein thrombosis and pulmonary embolism as follow-up events and can only be coded in patients with the procedure date on or after January 1st of 2020. If a patient has a DVT or PE event with a procedure date prior to January 1st of 2020, the event will be coded only as a non-device related readmission event if the patient was admitted for treatment. Some additional information. Based on the documentation provided, in this scenario, the follow-up events that would be captured are, number one, deep vein thrombosis would be captured based on the diagnosis of DVT with ultrasound confirmation. And two, a non-device related readmission would be captured based on the readmission that was unrelated to the LAA device. And lastly, pulmonary embolism would be captured for the PE diagnosed by CTA. Thank you for viewing the May 2021 case scenario.
