A very active 84-year-old female with a past medical history of sick sinus syndrome with an anticipated requirement for greater than 40% RV pacing is admitted for a CRTP. Multiple attempts to place the LV lead via the coronary venous system failed. Based on a shared decision-making conversation prior to the procedure, a HIS left bundle lead nor an epicardial lead would be attempted, therefore a dual-chamber pacemaker was implanted.
