This is the ICD Registry June 2020 monthly case scenario. We will review Section C, History and Risk Factors, Sequence 14731, Reason Pacing Indicated. Please note that this monthly case scenario was updated in April 2021 to reflect Version 2.3 changes. Our patient is admitted for a dual-chamber ICD for primary prevention. His past medical history includes ischemic cardiomyopathy and a prior myocardial infarction. His current New York Heart Association class is 2 with an LVEF of 28%. The patient is in sinus rhythm, however, he had episodes of Tachybrady syndrome associated with several episodes of syncope. How is Sequence 14731, Reason Pacing Indicated, coded? Is it sick sinus syndrome, complete heart block, chronotropic incompetence, Mobitz type 2, 2-to-1 AV block, anticipated requirement for greater than 40% RV pacing, AV node ablation, or heart failure unresponsive to guideline-directed medical therapy?
