Thank you for reviewing the AFib Ablation Registry Monthly Case Scenario for June 2021. This month's case scenario is focused on Sequence 10025, Discharge Atrial Rhythm. Our patient, Mr. Sanders, was admitted for an AFib ablation procedure, which goes on to be a successful procedure utilizing PVI as the ablation strategy. The rhythm documented immediately post-procedure in the recovery area was sinus rhythm. Mr. Sanders is discharged the day after the procedure. When abstracting the procedure, we find that an ECG was performed the morning of the discharge day in which the ECG shows a rhythm of AFib. This ECG is the last rhythm documented prior to the patient being discharged. Based on the documentation shared when abstracting the procedure into the data collection tool, how will we code Sequence 10025, Atrial Rhythm at Discharge? Should we code it as sinus, AFib, atrial tach, or not documented? Please review the documentation and question to determine the best answer. The answer is number 2, AFib. For the purpose of the AFib Ablation Registry, when coding Sequence 10025, Discharge Atrial Rhythm, please code the patient's last value, atrial rhythm, documented between the end of the procedure and discharge. In other words, please code the last atrial rhythm documented by a provider or nurse notes prior to the patient's discharge. If a 6-inch rhythm strip or ECG is part of the official medical record and it is the last atrial rhythm documented prior to discharge, as in this case, you may use the ECG in the coding of Sequence 10025. Some additional information. The coding of this element directly influences the results of the new informational data metrics that have been added to the Registry's dashboard. Metric 20, proportion of patients discharged in sinus rhythm. Metric 22, proportion of patients discharged in atrial paste rhythm. Metric 24, proportion of patients discharged in a rhythm other than sinus or atrial paste. For more information on these new metrics, please refer to the minutes of the Registry's May 2020 call.
