Thank you for reviewing the AFib Ablation Registry Monthly Case Scenario for June 2022. This case scenario will be focused on coding of baseline imaging performed, sequence 5170. Mrs. Perez had a successful AFib ablation procedure in which cryoablation was used as ablation strategy. While abstracting the procedure into the registry, the following history is documented in her medical record. Ischemic stroke, prior MI with the placement of a stent to the mid-RCA, diabetes, pneumonia, which required Mrs. Perez to be hospitalized for two months prior to the procedure. During that admission, we have documentation of a CT of the chest without contrast being performed and longstanding persistent AFib. Based on the documentation of a CT scan being performed within two months of the procedure, how will sequence 5170 baseline imaging performed be coded? Number one, no, or number two, yes. Please review the documentation and the question to determine the best answer. We will give you a few moments to review. The answer is number one, no. Based on the coding instructions, sequence number 5170 will be coded to indicate if pre-procedure imaging was performed. The target value for this sequence is last value between 90 days to the start of the procedure. Even though the patient had a CT performed within the target value, the CT was not performed as a pre-procedure imaging assessment, but to assess the patient's pneumonia. Sequence 5170 to 5190 will only be coded if the CT or MRI performed prior to the procedure was performed for the purpose of a pre-procedure assessment. For example, CT or MRI to assess cardiac anatomy prior to the ablation procedure. When sequence number 5170 baseline imaging performed is coded no, sequence 5175 baseline CT performed and sequence 5185 baseline MRI performed will gray out and not be available for coding.
