Welcome to the STS-ACC TVT Registry Case Scenario. Rather than the normal case scenario that is usually portrayed in this venue, we have decided to talk about the correct way to submit data to the DQR, or Data Quality Report, to avoid a submission error and avoid loss of follow-up entries on the dashboard. A frequent question sent in to the TVT Registry goes something like this. After my last submission to the DQR, my site is missing about one-third of my 30-day follow-up records and a few one-year follow-up records, but when I checked, the follow-ups have been entered into the registry. Can you tell me what has happened? To begin addressing this scenario, we'd like to first remind everyone of the correct process to submit data to the DQR. First, go to the TVT Registry homepage and locate the Data tab on the navigation bar, then access the Submit to DQR Version 3 tab. Once the Submit to DQR V3 page has been accessed, scroll down to the Base Submissions election and click Submit, the base first, and wait for the submission result, green, yellow, or red. If a green submission is achieved, then proceed to submit the follow-up. We do not recommend that the base and follow-up are submitted at the same time as an error may result if the follow-up is successfully processed before the base. To underscore one more time, if there are several quarters of data being submitted at the same time, please submit the base first and then submit the follow-ups. In addition, when the base is submitted or resubmitted, always submit or resubmit the follow-ups associated or linked to that quarter, even if the data has not changed. This will ensure the base and follow-ups are linked in the system. When the base submission quarter is the last, the base is unable to link the follow-up submissions that have been previously submitted. Said another way, the base cannot look backwards to find a follow-up. It can only look at submissions subsequent to the base. When the base quarter submission is last and the follow-up submission quarter or quarters were not resubmitted, follow-ups cannot link to the base data. For this reason, the follow-ups will not be seen in the numerator for metric 308, follow-up status dead or alive at 30 days, or metric 507, follow-up status dead or alive at one year. This is the reason that there is missing data from the follow-up timeframe, even if data is already submitted. The 30-day follow-up timeline is 23 to 75 days post-procedure. When submitting data, there is a high probability that there are patients who had their procedure in one quarter and their follow-up in the next quarter. In order to link those base and follow-ups so that they will appear on the same quarterly report, the current base and follow-up quarter need to be submitted along with the next quarter base and follow-up. So, how is the one-year follow-up calculated? The one-year follow-up provides information on patients who had their procedure five quarters back from the current data deadline to enable capture of all data submitted in the 305 to 425 days post-procedure timeframe. For a procedure that occurred on September 15, 2021, a follow-up could occur up to November 14, 2022, which is 425 days post-procedure.
