This case scenario involves a 37-year-old female with a past medical history of morbid obesity of greater than 400 pounds, long-standing sleep apnea, and atrial fibrillation.  Approximately one year ago, she underwent successful gastric bypass surgery with a weight loss greater than 150 pounds. The sleep apnea resolved with her significant weight loss.  However, her atrial fibrillation remains permanent despite multiple cardiovergence. Today she presents for a leadless pacemaker due to periods of near syncope caused by tachycardia  bradycardia syndrome, which was recently diagnosed.
