This is the IMPACT Registry July 2021 case scenario, PDA closure sequence number 7650, outcome of device. A three-year-old patient presents to the cath lab for PDA closure. The occluder is released into the defect, however, immediately after release, the device embolizes into the descending aorta and is successfully retrieved by sneering under fluoroscopy. The question is, how is sequence number 7650 outcome of device coded? Is it number one, implanted, not released, number two, implanted, released, or number three, implanted, released, and retrieved? Please take a moment to review the documentation and the question. And the answer is number two, implanted, released. The device was introduced into the patient and released into the PDA. When the device is introduced into the patient but subsequently removed during the procedure because the device either embolized or migrated, implanted, released, and retrieved would not be selected. Implanted, released, and retrieved would be coded when the device occluder is introduced into the patient and placed across the PDA, but because the device was either too small and or too large, it was removed. Implanted, not released, would be selected when the device is introduced into the patient but not released into the PDA. Implanted, released, would be selected when the device is introduced into the patient and released into the PDA. When the device is introduced into the patient but subsequently removed during the procedure because the device embolized, yes will be coded to sequence number 8055, device embolization, requiring device retrieval, and yes to sequence number 8060, retrieved via catheterization.
