This month's case scenario is focused on Sequence 7225, Intra-Procedure Anticoagulation, and Sequence 7250, Other. We have documentation of a patient being admitted for an AFib Ablation procedure. When abstracting the procedure into the registry, it is noted the patient's Eloquus was not held prior to the procedure, with the last dose of the medication being taken the morning of the procedure. It is also documented the operating clinician was made aware of the uninterrupted Eloquus therapy prior to the start of the procedure. Based on the documentation available, how will the uninterrupted Eloquus therapy be captured when coding the procedure's Intra-Procedure Anticoagulation strategy in Sequences 7225 to 7265? 1. Uninterrupted Warfarin therapy. 2. Other. Or 3. Bivalirudin. Please review the documentation and question to determine the best answer. We will give you a few seconds to review. The answer is number 2, Other. If Eloquus was not held for the procedure, please code Yes to Sequence 7250, Other Anticoagulant, as Eloquus, Apixaban, was not held for the procedure and therefore used as an anticoagulant during the procedure. The patient's medical record will reflect that therapy with Eloquus was continued and had not been held for the procedure. Sequence 7230, Uninterrupted Warfarin therapy, is to indicate if the patient continued on Warfarin therapy and it was not held for the procedure. It does not apply to P2Y12 inhibitors or NOAC agents. And Sequence 7245, Bivalirudin, is to indicate if Bivalirudin was used during the procedure. Apixaban will be coded as Current when capturing pre-procedure medications, as the medication was being taken routinely and was not held or discontinued prior to the procedure, with the last dose taken within 24 hours from the start of the procedure.
