Welcome to the STS ACC TVT Registry September case scenario. This month, we will discuss sequence number 13423, aortic valve calcification severity, and sequence number 13471, aortic valve annular calcification. During a patient workup for a TAVR procedure, CTA results show annulus minimum diameter of 20.1 millimeters, annulus maximum diameter of 38 millimeters, and AV annulus area of 300 millimeters squared. The echocardiogram reports severe aortic calcification. In this scenario, how is sequence number 13423 AV calcification severity coded? Is it number one, none? Number two, minimal? Number three, moderate severe? Or number four, left blank? Please take a moment to review the documentation and the question before making a final selection. When aortic calcification is documented by the echocardiogram and not available from CTA results, sequence number 13423 would be left blank. Instead, sequence number 13437, aortic valve calcification severity not documented, would be selected. Sequence number 13423 is part of section D, lab visit, under presentation and evaluation pre-procedure CTA findings. In this section, which is part of the full data set, all values are coded from the CTA findings. If the calcification value is not available on the CTA result, please code not documented. Please keep in mind that if your site is coding the basic data set, the pre-procedure CTA findings are completed at your discretion and carry a threshold of zero. If not completed, these data elements will not keep your facility from submitting or passing the DQR. Calcification identified on the echocardiogram is coded in section D, lab visit, under pre-procedure echocardiogram findings. When an echo report shows calcification, sequence number 13471, aortic valve annular calcification, is coded as yes. This data element is also part of the full data set and is completed at the discretion of your facility.
