Welcome to the STS-ACC TVT Registry October Case Scenario for Version 3. This month, we will cover Sequence 9002, Intra-Post-Procedure Events Occurred for Ischemic Stroke. A 74-year-old male presents for their TAVR procedure. The patient experienced multiple complications and it was noted the valve was not functioning as expected. Days later, he went to the OR to have the valve surgically removed. Post-operatively, he developed new-onset atrial fibrillation and some garbled speech was noted. Neurology saw the patient the next day and documented an ischemic stroke caused by Afib per the CT scan. Question, how is Sequence 9002, Intra-Post-Procedure Events Occurred, coded for ischemic stroke? Is it number one, no? Or number two, yes? Please take a few moments to review the documentation and the question prior to making a final decision. And the answer is number two, yes. First, let's review the target value. The term procedure in the target value, any occurrence between start of procedure and until next procedure or discharge for Sequence 9002, Intra-Post-Procedure Events Occurred, refers to a TVT procedure, TAVR, mitral replacement or repair, and or any catheter tricuspid procedure. This is supported in FAQ ID number 25345. The target value can be best understood if written as follows. Number one, any occurrence between start of procedure and until next TVT procedure within the same episode of care or number two, any occurrence between start of procedure and until discharge. Feel free to make a note of this in your data dictionary as well. For the version three upgrade, we are removing the language adverse events as this comes with a negative connotation assumed to be a direct result of the TVT procedure. These are now known as simply Intra- or Post-Procedure Events. Intra- or Post-Procedure Events are coded separately when they meet the specific data definition and target value. The STS-ACC TVT registry is interested in tracking all Intra- or Post-Procedure Events that occur between the start of the procedure and until the next procedure or discharge that meet the coding parameters, even though other surgeries, procedures, etc. may occur. This assures consistency in capturing data elements across the registry. Additionally, when capturing Intra- or Post-Procedure Events, the STS-ACC TVT registry does not attribute cause to the events.
