Welcome to the STS-ACC TVT Registry October 2022 case scenario. This month, we will discuss eligibility for follow-up metric 307, patients with acceptable quality of life outcome at 30 days based on the KCCQ summary score. In this month's case scenario, we will review five different scenarios and discuss each reason explaining why a record was not included in the numerator for metric 307, patients with acceptable quality of life outcome at 30 days based on the KCCQ summary score. But first, let's look at the numerator inclusion criteria. Eligibility for the numerator in metric 307 includes any TAVR patient who first had a 30-day follow-up performed within 23 to 75 days post-procedure. If this is met, secondly, the follow-up status in sequence 11004 must be coded as alive. If these are met, thirdly, the overall follow-up KCCQ must be coded with a score of at least 45 in sequence 14535 and have been performed within 23 to 75 days post-procedure and not post-discharge. Then lastly, the KCCQ overall summary score must not have decreased by more than 10 points from the pre-procedure KCCQ score coded in sequence 13843. Each of these points must be met for the patient to be included in the numerator. If or when patients do not meet the numerator, a review of the 30-day executive summary measures and metrics companion guide will aid in making this determination. The first patient did have a follow-up assessment performed in the correct time frame and the follow-up status was alive. However, in this scenario, the follow-up KCCQ overall summary score was not greater than 45 and the score decreased by greater than 10 points from the KCCQ overall in-hospital score. This patient did not meet two of the four inclusion criteria and would not be included in the numerator for metric 307. For patient number two, there was a follow-up performed within the specified time frame where the patient was coded as alive. However, the overall follow-up KCCQ score was not greater than or equal to 45 and does not meet the numerator inclusion. This example shows that the patient would not be included in the numerator for metric 307, even though only one of the criteria is not met. For patient number three, there was information entered for a follow-up where the patient was coded as alive for the follow-up status. However, the KCCQ was not performed or was not entered into the data collection tool, so the numerator inclusion is not met. In scenarios such as this one, if the KCCQ is found to have been performed, it may still be entered into the registry at any time, even if the data deadline has passed. For patient number four, the follow-up was completed with information that the patient was deceased. With a follow-up status of deceased coded in sequence 11004, the numerator inclusion is not met. For our last scenario, the fifth patient had their 30-day follow-up completed that identified the patient as alive in sequence 11004. However, the 30-day follow-up KCCQ date entered in sequence number 13844 was a date that occurred 79 days after the procedure, and it is out of the 23- to 75-day 30-day follow-up timeframe, leaving the patient out of the numerator.
