Thank you for reviewing the AFib Ablation Registry Monthly Case Scenario for November 2020. This month's case scenario is focused on Sequence 4110 and 4115, HAZ-Bled Drugs, Antiplatelet and NSAIDs. We have documentation of Mrs. Smith being admitted for an AFib Ablation procedure. Mrs. Smith's documented past medical history includes long-standing persistent AFib, ischemic stroke, and uncontrolled hypertension. Mrs. Smith's daily medication regimen includes metoprolol, amiodarone, as well as aspirin 162 mg. When abstracting the procedure and capturing the patient's HAZ-Bled risk score based on the patient's history and medication regimen, how will the aspirin 162 mg be coded? Please capture any dosage of aspirin as antiplatelet in Sequence 4110, HAZ-Bled, Antiplatelet. Sequence 4115, HAZ-Bled, NSAIDs, will be coded with traditional NSAIDs, such as ibuprofen, naproxen, and Celebrex. Answering yes on either or both of the medications will still only count for one point in the HAZ-Bled score. Based on Mrs. Smith's documented history, her HAZ-Bled risk score will be coded as yes to hypertension, yes to stroke, and based on the updated coding instructions, yes to antiplatelet. The coding instruction for these elements will be updated in the next data dictionary during the Registry's next upgrade.
