Thank you for reviewing the LAO Registry Monthly Case Scenario for November 2020. We will be discussing Sequence 4110, HazBled Drugs Antiplatelet, and Sequence 4115, HazBled Drugs NSAIDs. We have documentation of Mr. Jackson, who was admitted for an LAO procedure. Mr. Jackson's documented past medical history includes longstanding persistent AFib, CHF, ischemic stroke, and uncontrolled hypertension. Mr. Jackson's daily medication regimen includes metoprolol, amiodarone, warfarin, as well as aspirin, 162 milligrams. When abstracting the procedure and capturing the patient's HazBled risk score based on the patient's history and medication regimen, how will the aspirin, 162 milligrams, be coded? Will it be coded under Sequence 4110, Antiplatelet, or Sequence 4115, NSAIDs? Please review the documentation in question to determine the best answer. We will give you a few seconds to review. The answer is number 1, Sequence 4110, Antiplatelet. For the purpose of the LAO Registry, please code per the selection definition. Code yes if the patient currently uses or is prescribed aspirin or any other antiplatelet medication. This capture will be for any dosage of aspirin as antiplatelet in Sequence 4110, HazBled, Antiplatelet. Sequence 4115, HazBled, NSAIDs will be coded with traditional NSAIDs such as ibuprofen, naproxen, and Celebrex. Entering yes on either or both of the medications will still only count for 1 point in the HazBled score. You may also refer to FAQ ID number 25120. Based on Mr. Jackson's documented history, his HazBled risk score will be coded as yes to hypertension, yes to stroke, and based on the updated coding instructions, yes to antiplatelet. The coding instruction for these elements will be updated in the Data Dictionary during the Registry's next upgrade. Thank you for viewing the LAO Registry's case scenario for November 2020.

The case scenario involves Mr. Jackson, who has a medical history of longstanding persistent AFib, CHF, ischemic stroke, and uncontrolled hypertension, and takes medications including metoprolol, amiodarone, warfarin, and aspirin (162 milligrams). The question posed is how to code the aspirin in terms of the HazBled risk score. The answer is that it should be coded under Sequence 4110, Antiplatelet. The video provides further instructions on coding per the selection definition and differentiating between antiplatelet and NSAID medications. The HazBled risk score for Mr. Jackson includes hypertension, stroke, and antiplatelet use. The coding instructions will be updated in the Data Dictionary during the next upgrade.
