Thank you for viewing the LAAO Registry Monthly Case Scenario for December of 2021. We will be discussing how to fix follow-up error 1130. Our patient is a 78-year-old female who was admitted for an LAAO procedure. The procedure is successful with a 22-millimeter Amplatzer amulet device being implanted. The patient was discharged home the following day. Our 45-day follow-up assessment takes place 47 days after the procedure in the provider's office. The follow-up information is entered into the data collection tool by the abstractor, but when the follow-up information is submitted to the DQR, the submission receives a red inclusion status based on error 1130. Our question is, what does the abstractor have to do to clear error 1130? Number one, resubmit the follow-up entry. Number two, resubmit the base procedure entry. Or number three, delete the follow-up entry and reenter the data. Please take a moment to review the documentation and question to determine the best answer. We will give you a few seconds to review. The answer is number two, resubmit the base procedure entry. Based on system rules, every follow-up entry must have a base procedure that has been submitted and gone through the DQR process to link with. Error 1130 is produced by the system when a follow-up submission does not have a base procedure to link with. This can happen when the follow-up entry is submitted before the base entry or if both base and follow-up entries are submitted at the same time and the follow-up goes through before the base entry. In order to clear 1130, the base procedure entry of the patient receiving error 1130 must be submitted or resubmitted, followed by resubmitting the follow-up entry. Please do not submit together. Please allow the base submission to go through the DQR before resubmitting the follow-up. Error 1130 will cause a red benchmark inclusion status as the data assessment has failed. In order to find what error caused the submission to fail, we must click on the blue highlighted fail link. Clicking on the highlighted blue fail link will direct the participant to the data assessment report page, which lists all of the error and warnings of the submission. The error description will inform the participant of the patient causing the error and list the patient's demographics and their discharge date, which will inform which quarter the patient's base procedure belongs to. In this case, based on the patient's discharge date of August 3rd of 2021, the patient's base procedure belongs to 2021 quarter 3 base procedures, and that is the quarter that will need to be resubmitted. After the base procedure submission goes through the DQR process, the follow-up quarter with the error will need to be resubmitted. Having a base procedure to link to, the follow-up submission should pass the DQR process. For more information on the DQR process, please access the What is a DQR Rationale and Training course, available in the QII Learning Center under the Resources tab. To clear this error, the abstractor needs to resubmit the base procedure entry before resubmitting the follow-up entry. It is important to submit the base procedure first and allow it to go through the DQR process before resubmitting the follow-up entry. The video also provides information on how to find the specific error and provides access to further training materials.
