Welcome to the STS-ACC TVT Registry December 2021 case scenario. This month, we will cover questions from sequence number 13331, Anesthesia Type. A 75-year-old male presented for an elective transcatheter mitral valve replacement procedure. The case began with monitored anesthesia care of IV Versed and fentanyl. The patient did respond to verbal stimulation. During the course of the procedure, anesthesia was switched to total intravenous anesthesia due to patient movement. Once sedated with a continuous intravenous propofol infusion, the patient required assistance in maintaining their airway, but responded to repeated stimulation. Our question is, how would sequence number 13331, Anesthesia Type, be coded? Is it number one, general anesthesia? Number two, deep sedation analgesia? Number three, moderate sedation analgesia or conscious sedation? Or number four, minimal sedation anxiolysis? Please take a few moments to review the documentation and the question prior to making a final selection. And the answer is number two, deep sedation analgesia. The rationale being that an anesthesia type is coded to the highest value when multiple anesthesia types are used. It's coded on documentation of anesthesia type or descriptive documentation that meets the definition. In this scenario, the IV propofol caused a drug-induced depression of consciousness in which the patient was not easily aroused, but responded purposefully with repeated stimulation meeting the definition of deep.
