Welcome to the STS-ACC TBT registry case scenario for December 2023. In this scenario, we're going to review how to resolve a vendor error type with an 1130 or an 1140 error code in the DQR. In this scenario, a patient was admitted for a successful TAVR procedure and was discharged the next day. The patient returned for their follow-up and this information was entered into the data collection tool. When the follow-up quarter was submitted through the DQR, the file received a red submission for the benchmark inclusion status. When the fail link in the data assessment is clicked, the error informs the site a patient record had follow-up data which does not match the base submission mapping data as designated by the ACC for your facility or the patient was already reported to be deceased. Please contact your vendor for further assistance. In plain English, either number one, a follow-up was entered on the patient who was deceased at discharge and or number two, the link between base and follow-up has been broken. Most often, the 1130 or 1140 error code is produced when the follow-up entry submitted does not have a base procedure to link with. As well, the 1130 error code occurs when data has been updated in either the base or follow-up record and the link between this data is broken. It can also occur when the follow-up data has been submitted prior to submitting the base quarter. Far less often, the error can occur when the patient is deceased on discharge and a 30-day follow-up is submitted. It can also occur when a patient death is reported in the 30-day follow-up and a one-year follow-up is submitted. When looking at the difference between 1130 and 1140 error codes, an 1130 error code typically occurs when the link between the base and follow-up has been broken. An 1140 error code occurs when a follow-up has been resubmitted and the link between the follow-up and the base is broken. So, when an 1130 or an 1140 error code has occurred, how can it be resolved? Well, it will depend on whether the base and follow-up occurred in the same quarter or the case occurred in one quarter and the follow-up occurred in a subsequent quarter. To solve the error when the base and follow-up have occurred in the same quarter, go to the data tab on the navigation bar and choose submit to DQR version 3. Resubmit the base and get green status, then resubmit the follow-up. Take care not to submit both at the same time as it could cause the same error if the follow-up data is resulted prior to the base data. In order to demonstrate how the DQR is submitted, we will use 2023 quarter 2 as the base and 2023 quarter 2 follow-up. When the base has occurred in one quarter and the follow-up occurs in a subsequent, resolving in 1130 or 1140 error codes, it takes a few more steps. After accessing the submit to DQR version 3.0 tab, submit the base quarter and achieve a green submission status. Next, submit the follow-up for the same quarter and achieve either green or yellow status. Next, submit the base quarter for the next chronological quarter and achieve a green submission status. Then, submit the follow-up for the same quarter. Please keep in mind not to submit all quarters at the same time for the risk of follow-up data resulting prior to the base quarter data. In order to demonstrate how the DQR is submitted in this scenario, we will use 2023 quarter 2 as the base quarter and 2023 quarter 3 as the subsequent quarter. The order of submission to the DQR when the base is in one quarter and the follow-up is in the same quarter. Select 2023 quarter 2 base and achieve a green status. Then, select the 2023 quarter 2 follow-up and achieve either green or yellow submission status. Then, submit the subsequent base quarter of 2023 quarter 3 and achieve green submission status. And lastly, submit the subsequent follow-up of 2023 quarter 3. If the error persists, go to the follow-up and make sure that the patient did not have a follow-up entered for a patient death on the base procedure or on a prior follow-up record. To resolve this issue, delete the incorrect follow-up, rerun the quality check, and resubmit the data to the DQR for base and follow-up per the instructions on the previous slides.
