12. Improvement of Guideline Directed Medical Therapy Prescribed at Discharge for Patients Post Percutaneous Intervention

Using the National Cardiovascular Data Registry (NCDR®) CathPCI REGISTRY® data, a study was conducted to identify the inconsistent administration of Guideline Directed Medical Therapy (GDMT) post percutaneous intervention (PCI). Eisenhower Health (EH) had below-average outcomes in the CathPCI Registry for the prescription of guideline medications at discharge, including Aspirin, P2Y12, and Statin. Poor compliance with documenting contraindications to GDMT medications was also observed. The goal was to increase the prescription rates of these medications to improve cardiovascular outcomes and provider knowledge.<br /><br />To achieve improvement, a multidisciplinary team was formed, and the Plan-Do-Study-Act (PDSA) process improvement method was utilized. The team monitored and tracked CathPCI Registry data, conducted deep dives on outliers, and reviewed the data in committee meetings. Implementation steps included creating a hard stop on the post PCI order set to ensure providers addressed the medications, as well as providing education to providers and nurses. The project aimed to reduce potential re-admissions and improve compliance with documentation.<br /><br />The results showed significant improvements in metrics, with the prescription rates of guideline medications at discharge reaching 99.5% for Metric #38, 100% for Metrics #8, #9, and 99.4% for Metric #10 in Q4 2021 and Q1 2022. Patient safety was also enhanced, and there was a positive impact on patient recovery and overall health. The project helped EH demonstrate a commitment to providing excellent cardiovascular care and played a role in achieving the American College of Cardiology Cardiac Cath Lab Accreditation.<br /><br />In conclusion, EH implemented strategies to improve the prescription rates of GDMT medications post PCI, resulting in significant improvements in metrics and patient outcomes. The project highlighted the importance of adherence to guidelines and the value of providing high-quality cardiovascular care.
