1. While Waiting on Automation: A Nurse-Led Process in Promoting Cardiac Rehabilitation Referral, Enrollment and Participation
Back to course
Pdf Summary
This document discusses a nurse-led process for promoting referrals, enrollment, and participation in cardiac rehabilitation programs. The objective is to facilitate timely referrals from inpatient settings to outpatient cardiac rehabilitation before hospital discharge. The process is based on transformative design principles and incorporates evidence-based practice models. Referral communication is guided by CMS/ACC/ACVPR guidelines and measured rates of referral, enrollment, and participation.

The methods and materials used in this process include the IDEAS framework, Melnyk's EBP model, and the i-PHARIS framework. The data capture, analysis, and validity were evaluated through internal validation. Limitations include variations in data extraction methods and the suspension of data collection during the pandemic.

Results show an increase in referral rates and enrollment in outpatient cardiac rehabilitation programs. The referral rates among patients with coronary artery bypass, coronary stent, myocardial infarction, and heart valve surgeries increased from 3.7% to over 98%. The referral rate among heart failure patients with ejection fraction <40% progressed from 3.7% to over 90%.

The conclusion is that while waiting for referral automation, the referral communication process aids timely communication to providers in the community to support early enrollment in outpatient cardiac rehabilitation. However, the lack of referral automation with precision to eligible patient populations and supportive electronic medical record workflows remains a gap.

The document also provides contact information for Jonathan David, the nurse coordinator involved in the process, and acknowledges the contributions of other healthcare professionals.

Overall, this nurse-led process has shown promising results in promoting referrals, enrollment, and participation in cardiac rehabilitation programs. However, there is a need for further automation and improvement in referral processes to ensure efficient and effective care delivery.
