This study aimed to assess the efficacy of a new algorithm utilizing troponin measurements at 0 hours (Tr0hr) and 1 hour (Tr1hr) in risk stratifying patients with chest pain. The study population consisted of patients presenting to the emergency department (ED) with chest pain and complete troponin measurements. Data from the period before the algorithm change (July 2021 to January 2022) and after the algorithm change (February 2022 to August 2022) were compared.<br /><br />The results showed that the new algorithm led to varied changes in length of stay (LOS), but these changes were not statistically significant. However, there were trends indicating a decrease in the number of patients requiring a troponin measurement at 3 hours (Tr3hr). <br /><br />The adoption of the new algorithm incorporating Tr0hr and Tr1hr for risk stratification of patients with suspected acute coronary syndrome (ACS) has the potential to expedite patient disposition and decrease LOS. This could contribute to a decrease in hospital resource utilization and cost burden. However, the results of this study are limited by the short timeframe considered and the absence of a "blanking period" for clinical practice adjustment to the new algorithm. Inpatient presentations of chest pain were also excluded from the study.<br /><br />Overall, this study suggests that the new algorithm utilizing Tr0hr and Tr1hr has the potential to improve risk stratification in patients with chest pain and decrease patient LOS. Further research and longer-term analysis are needed to fully assess the impact of the new algorithm on hospital resource utilization.
