10. Developing My Personalized Hospitalization Account: The Patient Perspective

This project highlights the challenges faced by patients in understanding and following discharge instructions after hospitalization, particularly in cases of chronic conditions like heart failure. The study aims to improve patient satisfaction, reduce spending, increase medication adherence, improve transition of care and continuity, and decrease rehospitalizations.<br /><br />The findings reveal important opportunities for intervention, including addressing distrust in providers, delays in taking action, negative feelings about heart failure, negative behaviors, and suffering. However, patients were found to have positive knowledge and understanding about their condition.<br /><br />The study involved 53 hospitalized patients with heart failure who underwent semi-structured interviews, and content analysis was conducted to identify key themes. The study design is represented in a schematic, showing that the data analyzed for this project is focused on aim one.<br /><br />Overall, the study emphasizes the need for better communication and explanation of discharge instructions, as well as the inclusion of patient-specific information in the instructions. By gaining greater insight into patient challenges, understanding of their disease, and other social and economic factors, clinicians can improve patient care and outcomes. The study acknowledges the support of the UIC Linguistics, Cardiology, and Medicine Departments, as well as funding from the NIH for the project.
