15. Assessing Interventional Cardiology Physician Attitudes Following Implementation of Structured Reporting in the Cath Lab

This study assesses the attitudes of interventional cardiologists following the implementation of structured reporting (SR) in the cardiac catheterization laboratory (CCL). The benefits of SR in the CCL include improved reporting to the National Cardiovascular Data Registry (NCDR), more accurate capture of patient and procedural characteristics, decreased administrative costs, increased productivity, and improved quality and value. However, challenges during and after implementation include learning curve for new electronic health record (EHR) software, errors due to not removing default values, and increased report completion time initially.<br /><br />The project aimed to track the utilization of SR, the use of pre-populated choices instead of free texting, the most commonly used SR data elements, attitudes towards improved NCDR reporting and patient quality, perception towards improved reporting efficiency, and overall satisfaction. A structured electronic survey was sent to 18 interventional cardiologists across the system, with a focus on compliance, utilization, behavior, and attitudes.<br /><br />The results showed that 70% of interventional cardiologists utilized the structured template for cath lab reporting, and 20% tried to use pre-populated choices instead of free text responses. The use of SR was found to improve efficiency in the cath lab, accuracy of cath lab reports, communication with referring physicians, the ability to share data with national registries, and patient-centered quality metrics. Overall, the majority of interventional cardiologists were satisfied with the quality of SR format in the cath lab.<br /><br />The study suggests opportunities for further improvement, including having ancillary CCL staff enter as much information as possible to pre-populate the report, incorporating natural language processing software to capture essential data elements, providing feedback to show the benefits of electronic data uploading, and clarifying definitions for data elements in any SR to align with the CathPCI Registry.<br /><br />In conclusion, the study provides novel data on interventional cardiologists' attitudes and utilization of SR in the CCL. By addressing barriers and improving utilization, SR can lead to more accurate and timely reporting of cath data, improving quality, experience, and costs associated with manual abstraction/entry.
