Pdf Summary
The document discusses the implementation of a Cardiology Quality Program aimed at improving outpatient adherence to Guideline-Directed Medical Therapy (GDMT). The program utilized data transparency and pay-for-performance (P4P) incentives to engage clinicians in improving patient outcomes. The program included a multidisciplinary team and an online cardiology dashboard for clinicians to visualize their performance. Clinicians received quarterly reports and earned incentive amounts based on their performance. The baseline GDMT rates and clinician performance were established for each quality measure. The measures included beta blockers, ACE inhibitors, ARB/ARNi, MRA, statins, aspirin, and hypertension control. The program involved 53 cardiologists and assessed the care of 10,764 patients.

The results showed that the Cardiology Quality Program led to improvements in GDMT measures. The adherence rates for bisoprolol, metoprolol succinate, or carvedilol increased by 0.37%, ACE, ARB, or ARNi increased by 2.9%, MRA increased by 10.53%, statins for ASCVD prevention increased by 1.04%, aspirin for secondary ASCVD prevention increased by 1.88%, and hypertension control decreased slightly by 0.15%. These improvements were statistically significant for most measures.

The document highlights the importance of improving GDMT adherence in hospital and ambulatory settings and the variability of success with P4P programs. It concludes that the Cardiology Quality Program, combining P4P incentives with data transparency, was effective in improving GDMT adherence. However, further work is needed to optimize clinical workflows and continue improving adherence to GDMT.

Overall, the Cardiology Quality Program demonstrated that developing a quality infrastructure with data transparency and P4P incentives can improve GDMT in cardiovascular disease. The program engaged clinicians in understanding their data and patient panels, leading to better health outcomes for patients and lowering the risks of hospitalizations and mortality.
