Welcome to the Training Module 3 for ACC's Chest Pain Accreditation. This training module is designed for healthcare professionals who will take their facility through the chest pain accreditation process and review the data submission options, requirements for data submission, then finish with specific ACD and calculated measure training. If you are not using the ACD and using only the NCDR Chest Pain MI Registry, you may still benefit from slides 14 through 21, but this module is not required for NCDR Chest Pain MI Registry participants using only the registry for data submission requirements for the chest pain accreditation tool. The workshop modules are explicitly designed for any facility that is in the process of completing chest pain center accreditation. Any information regarding accreditation is highlighted in the workshop modules. The modules are subject to change at any time as we are continually updating the accreditation products to enhance the functionality of the tool for our customers. However, the facility's registered users of the online accreditation tool will have access to the most current workshop modules as needed. Anything shared in this training workshop module is confidential and privileged and only meant for the facility currently pursuing chest pain center accreditation. In addition, the modules and any information embedded cannot be published, distributed or shared with any third party. The objectives for this module is to first demonstrate how to utilize and operate the Accreditation Conformance Database, also known as the ACD. We will describe the ACD randomization program option for those that will continue to use the ACD as their data collection option through the application phase of the accreditation process. We will then review the calculated measures and their functionalities when using the ACD and lastly highlight all resources that are available to support these online chest pain center accreditation tool functions. Let's start with a review of the Accreditation Conformance Database. This workshop module will review the ACD and the calculated measure functionalities of the online chest pain accreditation tool. On the left menu panel, you can access the ACD or calculated measures list here. The ACD is not an electronic medical record and should only be utilized for the purposes of achieving and maintaining chest pain accreditation. The purpose of the ACD is to monitor clinical flow and daily patient level processes, but not meant to guide real-time therapy. It is designed to track the level of care provided and the patient movement through the continuum of care. The ACD was revamped to improve flow and user interface and will allow your facility to assess your compliance with mandatory items with minimal data abstraction efforts. We have removed the optional data elements from this chest pain accreditation model. The ACD Data Dictionary can be located under the Additional Resources tab. Before we begin, users may find it helpful to access and review the following guides, the ACD Guide and the ACD Data Dictionary. These guides can be easily accessed by going to the Additional Resources left menu section and then to the References Resources tab. For detailed ACD patient selection requirements, refer to the Patient Selection Criteria document under the References Resources tab within the Additional Resources. The Patient Selection Criteria document provides detailed definitions on the patient population and volumes that should be captured and included for data entry into the ACD. All patients captured into the ACD should be 18 years of age or older and have a primary ICD-10 discharge code listed on the Patient Selection Criteria Guide. To access the ACD, click on the ACD on the left navigation bar menu of the Chest Pain Accreditation online tool. This will then open the ACD Patient Summary page. The table in the main body of the page will populate with any ACD patient encounters that have already been entered. In this example, there are several patient encounters that have already been entered into this facility's ACD. To edit an existing ACD patient encounter, click on Details to enter or review that patient record. On the top left-hand side, there is an option to return to the main Chest Pain homepage by clicking on Return to Tool. Now moving to the far right side, there is a three-line icon. Click on Help to access the ACD Guide. Selecting the Chest Pain Center Data Collection Form provides a blank ACD data collection form in PDF format to facilitate data collection. Selecting the Measures Year-to-Date Report will take you to your facility's current year-to-date metric summary report. To enter a new patient into the ACD, select the Add Record button. To change the patient summary range, click in the Start Date box to change, then click Refresh. When adding the new patient episode of care, the next screen will require the patient ID, encounter ID, and date and time of arrival. Once entered, click on the Create Record button. In addition, there is helpful information available embedded within the ACD. Remember that only the facility can see the patient medical record number. You must use the lookup code for communication with your accreditation review specialist. For example, if the blue question mark icon is selected beside a data element, a pop-up window will appear with additional information regarding that data field. To delete a record, click on the three-line icon and then select the Delete option. Selecting the Delete button will redirect to a confirmation window where verification to delete the record occurs in two locations. Check the small box on the upper left, Are you sure you would like to delete this record? Then click the Delete Record button.
