The procedure information section is completed for each lab visit. The intent of sequence 15694, Procedure Room Entry Date and Time, is to capture the date and time the patient physically entered the procedure room according to the medical documentation. Sequence 15695, Procedure Room Exit Date and Time, then captures the date and time the patient physically exited the procedure room. Collecting procedure room entry and exit times will provide the opportunity for benchmarking comparisons of throughput measures within the registry. Sequence number 15786, Post-Market Surveillance, identifies if the ICD or pacemaker, generator or lead is part of a post-market surveillance clinical trial. Name, NPI number, and the fellowship training program have been added as optional fields. Indicate the name of the fellow in training as well as their National Provider Identifier number and the institution's Accreditation Council for Graduate Medical Education number for the program in which the fellow is participating.
