Section G, Device Implant-Explant, will be broken down into multiple presentations. Let's start by discussing primary tachycardia indication present, sequence 14729, bradycardia indication present, sequence 14730, and primary bradycardia indication present, sequence 14737. This entire section, which began with indication for permanent pacemaker, sequence 4330, has changed. Let's look at each section individually. Primary tachycardia indication present, sequence 14729, indicate if there was a documented primary tachycardia indication for the ICD implantation based on the value on current procedure. For further clarification, a primary tachycardia indication is asking, is there a reason the patient requires a defibrillator? Are they at risk for having sudden death or have they already experienced an episode of sustained VT, defib, or sudden cardiac arrest? In other words, is there a documented primary or secondary ICD indication for the device? Class one or two guideline bradycardia pacemaker indication present, sequence 4335, has been retired. Those patients receiving an ICD on current procedure will have a primary tachycardia indication or reason for getting the defibrillator. We also want to know if they require permanent pacing, which would be a secondary reason for the device. For patients not receiving a device with a defibrillator, select no for primary tachycardia indication. Those patients receiving a permanent pacemaker on current procedure will have a primary bradycardia indication or reason for getting the pacemaker.
