Before we look at the abnormal, let's talk about normal conduction. Normal conduction begins with the activation of the SA node and transmits the signal across both the right and left atrium, resulting in atrial contraction. The signal then travels to the AV node. At the AV node, there is a delay of about 120 milliseconds. Before the signal travels through the bundle of His, and splits into the right and left bundle branches. The left is activated slightly before the right. Next, the left bundle splits into the left and anterior fascicles. From there, the signal spreads into the Purkinje fibers of the left and right ventricle, resulting in ventricular contraction. SA node dysfunction, or AV node disease, can lead to a disruption in the conduction system of the heart requiring the implantation of a permanent pacemaker.
