Section F procedure information has several new data elements and updates. All elements highlighted in yellow indicate a change from version 2.2. ICD indication sequence 7015 is for initial ICD or CRTD generator implants and generator changes. It is no longer required for ICD or CRTD generator implants. Shared decision-making sequence 14732 seeks to capture documentation in the medical record indicating a shared decision-making encounter occurred for a primary prevention procedure to meet the target value on current procedure. This data element is intended for all initial primary prevention procedures. However, it is also available to capture for all procedures as shared decision-making has been shown to be beneficial for all procedures, not just primary prevention ICDs. Shared decision-making is when patients and clinicians work as a team to make care decisions. Tools can help facilitate a collaborative process between provider and patients and can increase knowledge and satisfaction regarding care, define clear goals for treatment, and align health care decisions with patient values. Shared decision-making tool used sequence 14733 indicates that a shared decision-making tool was used to facilitate the conversation between the clinician and the patient on current procedure. When yes is coded, please indicate the shared decision-making tool name in sequence 14734 on current procedure. If the tool used is not in the drop-down list, please contact NCDR to request the selection be added. Shared decision-making is required by the Centers for Medicare and Medicaid Services National Coverage Determination for Primary Prevention Initial ICD Generator Implants. For Indications 2 through 5, a formal shared decision-making encounter must occur between the patient and a physician or qualified non-physician practitioner PA, NP, or CNS using an evidence-based decision tool on ICDs prior to initial ICD implantation. Please do note the shared decision-making encounter may occur at a separate visit. Shared decision-making is in the guidelines and as shown here is a class 1 recommendation for the implantation of an ICD for primary or secondary prevention of sudden cardiac death.
