Welcome to Lesson 1 of ACC's Heart Failure Accreditation Tool Orientation Course. This learning activity was developed by myself. I am Liza St. Clair, and I will also be narrating this presentation. Our objectives for this lesson is to first demonstrate how to navigate the online Heart Failure Accreditation Tool and identify essential information from your team that must be entered into the facility profile of the tool. Once logged into the Heart Failure Online Accreditation Tool, the first page shown is the home page. On the left-hand side, there is a navigation bar which allows access to your facility information, contacts, your essential components, calculated measures, supporting documents, submission of your baseline and application phase, the message center, reports, contact us, additional resources, and your Accreditation Conformance Database, and as well the ability to change your password. This is where navigating will provide access to all elements of the tool, resources, references, and the ability to communicate with your assigned Accreditation Review Specialist. Before we begin, viewers are highly advised to review the user guides that will assist and facilitate the learning of the topics discussed in this workshop module, which are found under Additional Resources. To access the guide, click on References Resources tab. There, users can access the following guides, the Tool Guide and the Messaging Center User Guide. These documents complement the topics that will be reviewed and provide more comprehensive overview of the areas that will be covered within this module. Therefore, it will be helpful to have these guides available as a reference while completing this module. The top header of the homepage provides the user access to several functions and outlines the facility's progress towards accreditation. In navigating the header, the top left provides your facility account number and the facility name, and the Action feature allows those facilities participating in multiple ACC Accreditation Service Lines to toggle to those other service lines or other online accreditation tools. Consult the Tool Guide for additional features. On the top right-hand side of the homepage, there is other useful information. The username is displayed along with the option to log out to the right of the username. The other features are the days left for the next submission and submission progress bars to track and monitor the progress towards deadlines. Reminders are provided should the facility become past due for submission, as shown in this example. Once the facility completes all accreditation phases, the progress bars will reflect a numeric representation as in this image, shown as 100% completed in both submission progress bars. Below the top header of the tool, the title of the navigation bar will be displayed. For ease of navigation, there are Back and Next buttons on each of the main pages of the Heart Failure Accreditation Tool functions. Back and Next buttons will allow the user to toggle through all sections of the Accreditation Tool. This function directly corresponds to the options provided on the left navigation bar. Therefore, the user may use this method as well to suit their navigation preference. The Print function will allow easy access to review the Essential Component Completion Report. This may serve as a helpful tool to keep track of the completed areas and tasks delegated amongst individuals within the heart failure multidisciplinary team. The Print feature will provide the option to select the EC sections to include within the report. For the purposes of this training, we will click on Select All. This will autofill all the checkboxes below. The PDF report can now be downloaded. Depending on each individual computer configurations, the report may download automatically or it may require selecting the Open With option to save. Here is an example of the report. Note the feature on the right hand side. It provides a complete column which will indicate the status of that particular EC line item. In addition, while navigating the Accreditation Tool, a facility can easily reference the Online Accreditation Tool Guide by selecting Help. The first step to complete when accessing the Online Heart Failure Accreditation Tool is to complete your facility profile. To do so, click on Facility Information and enter the requested information within the submenus. Please enter and edit the requested information. At the bottom of each submenu page, confirm that the information is correct. And upon selecting Yes, this will activate a green checkbox on the left navigation bar. This indicates that the information required for this section has now been completed. This green checkmark feature denotes completion of a specific area, allowing for quick visualization of which items still require attention. The Facility Foundation information will record the facility's declared data option. Data option will be captured within the Facility Information submenu. By providing flexible data options, it decreases the data burden and allows accreditation services to adopt any heart failure data collection process that may already be employed at the facility. To comply with the required performance measures within the essential components, all sites will need to demonstrate compliance with these performance metrics by utilizing the Accreditation Conformance Database and or providing supporting documentation from other data sources. The Accreditation Conformance Database is the first option, which is a data collection tool already built within your online heart failure accreditation tool. Therefore, there is no need for a facility to create their own database or to collect their own heart failure data. The next two options is the National or Regional Database or Registry or a facility electronic health record or database, which is defined as external metrics which demonstrate the facility meets the mandatory performance metrics stipulated in the Essential Components and Clinical Quality section following the baseline phase. Please check all that may apply to your facility. For the purposes of this demonstration, we will select all three of the options. In any case, when an outside data source is utilized for the purpose of achieving accreditation, please record the name of the data source in the appropriate data fields. Next, for facilities opting to use Accreditation Conformance Database, the facility will need to do one additional field. Within your facility foundational information, the facility must elect which version of the ACD that will be utilized. Depending on which option was chosen will determine which data fields within the ACD must be completed. Although the limited ACD option allows for minimal data abstraction to meet the mandatory Clinical Quality Essential Component requirements, all facilities are encouraged to utilize the full ACD data set, as this option will provide a more robust set of performance metrics to analyze and provide a greater opportunity of insight and reflect the true operation of the facility's heart failure program. Next, the facility should confirm their contacts. The left navigation bar, click on Contacts and enter and confirm key contacts within the organization. At a minimum, the following must be provided, Executive Sponsor, Key Contact, CEO or President, and the Heart Failure Administrative Coordinator or Navigator. Fill in the appropriate fields and fields with the red asterisks must be completed. The functionality of the Essential Component section will be specifically reviewed in Lesson 2 of this course. Features of the Accreditation Conformance Database and Calculated Measures reviewed in a separate Heart Failure Accreditation QII Learning Center course. Moving down the left navigation bar, the next selection is the Shared Practices option. Selecting this function will provide a consolidated list of examples of other ACC Heart Failure Accredited Facilities supporting documents. Facilities may utilize these templates as guidance to create a framework of forming their own specific policies or protocols. Supporting documents are uploaded after the baseline phase. After the Baseline Gap Analysis has been completed, the facility can select this function to add, review, or delete any supporting documents that are required to demonstrate compliance with the Essential Component line item. Once all components of the Baseline Gap Analysis are completed, the facility will go to the Accreditation Tool homepage and select Submit Baseline on the left navigation bar. Once baseline submission is complete, the Submit Baseline selection on the left navigation bar now changed to Submit Application indicate that the facility has completed the first phase of the accreditation process. Now the facility is at the beginning of the second phase of the accreditation process, the notification phase. After entering a message, it will populate the Message Center Notification box found on the left hand navigation column of the Online Accreditation Tool. This allows for a quick visual to see that a response was provided or a message was sent by the ARS. Message Center lists all current communication items in descending order from the most recent and will allow the user to reply directly to the ARS. As noted earlier, for further details on how to use the Message Center, please refer to the Messaging Center How-To Guide under the References and Resources tab. Facilities electing to utilize the ACD, validation, process measures, and general informational reports are available in the Reports function. Information in the calculated measures will be reflected in the Measures Year-to-Date Report and provides a monthly view of the submitted information. During the accreditation process, the Measure Year-to-Date Report will likely be referenced often to validate the data entry. Data reflects erroneous information such as negative numbers or results well outside of the range. Changes can be made to the patient record as needed. Changes will then be updated and reflected in the report by the next day. This is a sample of the Measure Year-to-Date Report. The initial screen reflects the monthly data per calculated measure. On the right hand side of the report, the date and time of when the report was last updated will be displayed. Scroll down on the patient episodes of care that qualified for the measure and click on Details under the Calculated Measure Identifier. A table will then display with detailed information. Users may now analyze and validate the cases that were included in the measure and determine how the calculated measure value was computed. If a user would like to review the patient record of any outliers or make any changes to a patient record, the user simply needs to click on the View hyperlink on the right hand column to view the patient's ACD record. The Measure Definition Report provides a report of the calculated measures with measure definition and the different components related to the measure, such as linked essential component requirements, inclusion and exclusion criteria, and how the measure was computed. The Essential Component Completion Progress Summary Report is similar to the EC Completion Report available on the homepage. However, this report is more comprehensive and allows the user to review both completed and uncompleted essential component line items. When the completion rate is not 100%, specific values are highlighted in red font. These values are direct hyperlinked to a drill-down report of those specific essential components that calculated that value. Lastly, there is the Guidance Statement Report. This report provides a printable or downloadable list of all essential components with guidance statements. For further assistance or questions regarding the Heart Failure Accreditation Processor Tool, click on Contact Us on the left navigation bar. The central telephone number is provided and two hyperlinks to two different email addresses are available as well. Contact Accreditation Services at accreditationinfo.acc.org for new product purchase questions. And when a facility does not have an assigned ARS yet, the facility may request technical support or ask clinical questions with the second option provided, which will direct the facility to the accreditationsupport.acc.org email address. Click on the Additional Resources selection on the left navigation bar to access all general resource documents. There are frequent additions to the information within this section of the tool. Information on updates to any of these tabs will be provided as needed. The References Resources tab houses a compendium of user guides and other additional resources. Under the Educational Webinars tab, there are past webinars that can be viewed by the user. The Updates tab documents and archives any release notifications regarding any upgrades or changes that may have been made to the online tool. The Workshop tab provides access to training modules on topics related to the accreditation tool. And lastly, the ACC Learning Center tab will allow you to access these accreditation workshop modules and other educational offerings within ACC's QII Learning Center. The advantage of this learning option allows the user to obtain C&E credits. Lastly, there is an option for the user to change their password when accessing the accreditation tool.
