Welcome to Lesson 4 of ACC's Heart Failure Accreditation Tool Orientation Course. This learning activity was developed by myself. I am Liza St. Clair, and I will also be narrating this presentation. This module is specifically designed for reaccrediting facilities and to review several new optional features within the Heart Failure Accreditation Tool that may be utilized. The objectives are to define the new functionalities within the tool, which will include bypassing the gap analysis, which will include entering your foundational information, contacts and essential components, porting documents over from the previous accreditation application, and lastly, the attestation option for select essential components. Bypassing the Baseline Gap Analysis Phase. After achieving your Heart Failure Accreditation Award, sites may opt to bypass the Baseline Gap Analysis Phase, but prior to selecting this option, let's review how this impacts your reaccreditation journey. This feature will apply to reaccrediting facilities only. When utilizing the term bypassing the Baseline Gap Analysis, we are referring to three sections. Autofilling of the foundational information and EHR, autofilling of your contacts, and bypassing your initial assessment of compliance with the essential components. The next few slides will review the bypass BGA features. Again, this feature is only available for reaccrediting facilities. If your facility is seeking their initial accreditation, this option will not be available until 12 months prior to the expiration of your accreditation. To best describe this process, we will start from the homepage or landing page. We will zoom in on the menu to the left of the page. For reaccrediting facilities that were previously enrolled in ACC's Heart Failure Accreditation Program, there is an option to copy over the previous version of the facility's foundational information, electronic health record, and contacts. However, all information should be verified. For contacts, only information that has passed with a green checkmark can be copied over. Now that we are zoomed in on the menu, we will select Facility Information. From there, we will click on Foundational Information, which will then provide the option to copy the information from the prior accreditation. Clicking on Yes will copy the information from the previous accreditation application and serve as your acknowledgement that the information remains up-to-date. Clicking on No will provide the option to enter the information manually. Following the same process, we will now select Electronic Health Records. The same option will present itself under this section. Make your selection here, just as was done in the previous section. You can see you will follow the same process in completing the contact section as well. Click on each contact and make your selection as appropriate. This specific contact was not filled in previously. No information will transfer, and information will need to be added as appropriate. For existing facilities that were previously enrolled in Heart Failure Accreditation, there is an option to copy over the previous version of each facility contacts. Again, this will only copy over the information previously provided and had a green checkmark. Options are as follows. Copy information from previous version, which are correct and validated. This option will copy over all information only if previously available and had the green checkmark. Fields will auto-populate and can be updated as needed. For each contact, confirm the checkbox at the bottom of the screen. I have reviewed the above information and made all necessary corrections to activate the green checkmark. The second option is no, do not copy updated information, will be entered. This selection will not copy over any information if it was previously available. This selection is for facilities who elect to manually enter all sections. For each contact, please confirm and check the box at the bottom of the screen. I have reviewed the above information and made all necessary corrections, which activates the green checkmark. After navigating the Facility Information tab and the Contacts tab, the next tab on the list is Essential Components. Clicking on Essential Components, a pop-up window will appear asking whether you would like to bypass entering the essential components. Clicking yes will auto-answer each one with an NA. Clicking no will default to the pathway of answering each essential component to complete the Baseline Gap Analysis. Historical Supporting Documents For facilities renewing their accreditation, documents uploaded in the previous accreditation will automatically port over to the new accreditation application. To locate them, simply click on Supporting Documents from the left-sided menu. As you can see, there are two tabs here, one for current documents, which are documents uploaded during the current accreditation application, and another for historical documents, which are documents uploaded during the previous accreditation. Only documents in the Approved Status or Unread Status will port over to the new application. The historical documents are available as a reference and cannot be attached to any EC in the current application. Attestation Option for Essential Components Once a re-accrediting facility moves beyond the baseline phase and into the application phase, the ability to attest to designated essential components is available. Looking in Governance at EC1 M1A, you can now view the Attest as Current column here. By clicking on the Attest as Current caret, this brings up a drop-down menu. By selecting Yes, you are stating or attesting the document uploaded in the prior accreditation application still demonstrates current practice without changes. This will place an NA in the Currently Met column. There will be no need to upload a supporting document for this EC unless requested by your ARS. Looking at the same EC, selecting No to the Attest as Current section will simply take you along the normal pathway. Having to answer Yes or No to the Currently Met column will require you to upload a supporting document within your application phase. Finally, looking at EC1 M1C, you can see that this is one of the ECs designated as not attestable. These ECs will be grayed out in this section with an NA. You will simply follow the normal pathway in completing the ECs, answering Yes or No, and upload a supporting document. This concludes Lesson 404. You have now completed all lessons.
