Hello, my name is Nicole Roman and I am an Accreditation Review Specialist with American College of Cardiology Accreditation Services. This lesson will show you how to upload a CSV file into the Accreditation Conformance Database, also known as the ACD. The objectives for today's lesson include how to identify what the CSV file is, figuring out where the CSV template is located within the ACD, determining how to correctly format your CSV file, and demonstrating how to upload the CSV file. So what is a CSV file? A CSV is a type of Excel worksheet saved as a comma-separated value file. If your facility has high enough volumes to randomize your data, submission of the CSV file is what is used for this encounter randomization and is required monthly into the ACD. Your CSV file must be formatted exactly like the template given in the ACD. So where can you find the CSV file template? You are first going to log into the Accreditation Tool and then go to the ACD. Once you are within the ACD, click on the three bars in the upper right-hand corner. You will select Upload Sample File and then click on Download Template CSV, which is in the upper right-hand corner. If you are applying for chest pain accreditation, this slide is for you. Once you have downloaded the template, format your report exactly the same way as the template is formatted. You will want to make sure that your report only includes final primary discharge diagnosis of the ICD-10 codes listed on the screen. For example, if the patient's final primary discharge diagnosis is pneumonia and the secondary discharge diagnosis is chest pain, you would exclude that patient. You should also exclude all elective cardiac cath procedures as well as outpatient and abed status patients. As a side note, all STEMIs are entered manually into the ACD and are not part of the CSV file. If your document has additional information or is missing information, the ACD may reject the file. You'll want to make sure to save your file as a CSV and not an Excel. If you are applying for heart failure accreditation, this slide is for you. Once you have downloaded the template, format your report exactly the same way as the template is formatted. You will want to make sure that your report only includes final primary discharge diagnosis of the ICD-10 codes listed on the screen. For example, if the patient's final primary discharge diagnosis is pneumonia and the secondary discharge diagnosis is heart failure, you would exclude that patient. You should also exclude outpatient and abed status patients. If your document has additional information or is missing information, the ACD may reject the file. Make sure to save your file as a CSV and not as an Excel. Your CSV file should appear like this example seen here on the screen. As a reminder, if it is not formatted in the exact same way as the template, the ACD might reject your CSV file. Once your report is ready and you are sure it is including as well as excluding the correct patients, you can now upload your CSV file. Once logged into the tool and once you're in the ACD, click on the three bars in the upper right-hand corner and select Upload Sample File. Once you're on this screen, select the sequence you will be abstracting. As seen in this example, if you are abstracting May data, that corresponds to sequence number two. After you have selected sequence number two, you will then click on Select Files. This will open up a little box and you will select the correct file from your computer to upload and click Open. You will see that your file has been attached, then simply click Upload. You will see a spinning wheel which may take up to a few minutes and this is completely normal. If the upload is successful, you will get a message about records added. At that point, click on Return to Patient Summary in the upper left-hand corner to return to the ACD main screen to see all the randomized records which were added. You are now ready to start abstracting. This concludes our mini-module lesson on how to upload a CSV file into the Accreditation Conformance Database, also known as the ACD.
