Hello, everyone, and welcome back to our bi-monthly case scenario for April 2025. For this scenario, we will be discussing the follow-up submission deadline schedule and how to understand it better. Based on participant feedback and the questions received regarding this topic prior to the last data deadline, we thought it would be helpful to create a resource to better explain the data submission schedule, especially the follow-up submission schedule. Call for Data Schedule is found under the Data tab and Call for Data Schedule. The Call for Data Schedule includes data submission deadline. For this case scenario, we are using the next deadline, which is Thursday, June 5, 2025. The Base Procedure Quarter Due, which in this case is 2025 Quarter 1, includes base procedures with a discharge date from January 1 to March 31, 2025, and the follow-up submission schedule, which we will be covering today. Before we begin, there are some important factors to remember when talking about follow-up submissions. Number one, a follow-up entry will be part of the quarter in which the assessment date coded corresponds. For example, if the assessment date coded in the follow-up is between January 1 and March 31 of 2025, the follow-up will be part of 2025 Quarter 1 follow-ups, regardless of the discharge or procedure date. Number two, when reading the follow-up submission schedule, we are looking at the follow-ups due based on the Base Procedure Quarter, which are based on the discharge date. If the discharge date is between October 1 and December 31 of 2024, the base procedure will be part of the 2024 Quarter 4 base, regardless of the procedure date. For example, if the procedure is performed on September 29 and the patient is discharged on October 1, the base submission for this patient will be part of Quarter 4 based on the discharge date. Let's begin with the 45-day follow-ups that are due based on the June 5 deadline. When looking at the schedule, the 2024 Quarter 4 under the 45-day follow-ups do not mean that only 2024 Quarter 4 follow-ups will need to be submitted. What it means is the follow-ups due are the 45-day follow-ups of procedure with discharge dates in 2024 Quarter 4. Knowing this, when submitting the 45-day follow-ups due, based on the procedure date and follow-up time frames, these follow-ups will most likely take place between 2024 Quarter 4 and 2025 Quarter 1, which lets us know which follow-up quarters should be submitted. Of course, this may be different based on the facility's population and the follow-up assessment dates. For example, if all the 45-day follow-ups for procedures taking place in 2024 Quarter 4 took place between October and December of 2024, then only 2024 Quarter 4 follow-ups will need to be submitted. Let's continue with the 6-month follow-ups that are due based on the June 5 deadline. When looking at the schedule, the 2024 Quarter 2 under the 6-month follow-ups do not mean that only 2024 Quarter 2 follow-ups will need to be submitted. What it means is the follow-ups due are the 6-month follow-ups of procedures with discharge dates in 2024 Quarter 2. Knowing this, when submitting the 6-month follow-ups due, based on the procedure dates and follow-up time frames, these follow-ups will most likely take place between 2024 Quarter 4 and 2025 Quarter 1, which lets us know which follow-up quarters should be submitted. Of course, just like with the 45-day follow-ups, this may be different based on the facility's population and the follow-up assessment dates. Continuing with the 1-year follow-ups that are due based on the June 5 deadline. When looking at the schedule, the 2023 Quarter 3 under the 1-year follow-ups do not mean that only 2023 Quarter 3 follow-ups will need to be submitted. What it means is the follow-ups due are the 1-year follow-ups of procedures with discharge dates in 2023 Quarter 3. Knowing this, when submitting the 1-year follow-ups due, based on the procedure dates and follow-up time frames, these follow-ups will most likely take place between 2024 Quarter 3, 2024 Quarter 4, and 2025 Quarter 1, which lets us know which follow-up quarters should be submitted. Of course, just like the other follow-ups, this may be different based on the facility's population and the follow-up assessment dates. And finally, the 2-year follow-ups that are due based on the June 5 deadline. When looking at the schedule, the 2022 Quarter 3 under the 2-year follow-ups do not mean that only 2022 Quarter 3 follow-ups will need to be submitted. What it means is the follow-ups due are the 2-year follow-ups of procedures with discharge dates in 2022 Quarter 3. Knowing this, when submitting the 2-year follow-ups due, based on the procedure dates and follow-up time frames, these follow-ups will most likely take place between 2024 Quarter 2, 2024 Quarter 3, 2024 Quarter 4, and 2025 Quarter 1, which lets us know which follow-up quarters should be submitted. Of course, just like with the other follow-ups, this may be different based on the facility's population and the follow-up assessment dates. Let's go through some bonus questions. When looking at the data submission schedule for the dated deadline of June 5, 2025, we see that for the 45-day follow-ups, the follow-ups that are due are 2024 Quarter 4. Does that mean the follow-up quarter we need to submit is 2024 Quarter 4? Number 1, yes. Or number 2, no. The answer is number 2, no. It means the 45-day follow-ups that are due are 45-day follow-ups of base procedures with discharge dates in 2024 Quarter 4. Again, this means that if a procedure took place on a later date in 2024 Quarter 4, the 45-day follow-up most probably was performed in 2025 Quarter 1, meaning 2025 Quarter 1 follow-ups will also need to be submitted in order for that patient's 45-day follow-up to be aggregated by the deadline. A second bonus question. At the data submission schedule for the deadline of June 5, 2025, we see that for the 6-month follow-ups, the follow-ups that are due are 2024 Quarter 2. A patient has their procedure on June 18, 2024, and is discharged on June 20, 2024, and had their 6-month follow-up on January 2, 2025. In order to meet the deadline and submit this patient's 6-month follow-up, do we submit 2024 Quarter 2 or 2025 Quarter 1 follow-ups? Number 1, 2024 Quarter 2, or number 2, 2025 Quarter 1? The answer is number 2, 2025 Quarter 1. In order to submit this patient's 6-month follow-up, we need to submit 2025 Quarter 1, since the follow-up took place January of 2025.
