A 67-year-old man underwent a successful LAAO with an Amulet device, but follow-up TEE showed an enlarging residual leak. He later had a second registry-eligible percutaneous LAAO procedure using an Amplatzer Duct Occluder to repair the leak, with the leak reduced to zero. The second procedure is captured in the registry as a new episode of care under the same patient. It should also be reported as a follow-up event for the original LAAO, with reintervention, unplanned intervention, and device-related readmission coded at the six-month follow-up.
