Video Summary
An 82-year-old man underwent elective TAVR complicated by valve migration and a type A aortic dissection. The valve was recaptured and deployed successfully despite the dissection, with no immediate hemodynamic compromise. After confirming the ascending aortic dissection, the patient underwent CTA and then was taken to the CVOR for surgery. Sequence 13-542, conversion to open-heart surgery, should be coded “Yes” because open-heart surgical access was required during the current procedure, even though CTA occurred first. This also affects metrics, including significant cardiac event, and several complications were captured: device embolization, aortic dissection, unplanned cardiac surgery, re-intervention on the aortic valve, and major vascular complication.
