Welcome to this learning activity titled Start Here. Upon completion of this four-part learning activity, the learner should be able to utilize the Registry Site Manager and individual user checklists to ensure successful registry setup, create registry resource documents, and identify the location of the 10 steps to registry success. What is the difference between the Coder's Data Dictionary and the Full Specification Data Dictionary? Why is it important to note the last modification date of a document? The Resources link essential information portion of this webinar will answer these questions and more. Data collection forms as well as important documents are in the Resources link. To locate registry documents, navigate to the Resources link in the left navigation bar. Select Documents, and every in-hospital registry will have the following document categories. Data Collection Documents, User Guide Documents, Quality Tools and Reference Documents, NCDR Risk Model Documents. Additionally, the registries will have registry-specific documents. Documents in every document category will have important information for the user to note. The file or document name, the format or type of the file, the size of the file, the date of last modification, which is important to note to ensure the downloaded document you are using is the most current. The blue text download is a hyperlink to open and download the file or document related to the registry. Additionally, for your convenience, the document's homepage includes a hyperlink to the NCDR Registry Program requirements. The Data Collection Documents section is primarily used by data abstraction personnel. The Data Collection form mirrors the data elements in the Electronic Data Collection tool and is provided in two formats, PDF or Word. Both formats can be downloaded or printed for use during the data abstraction process. The Coder's Data Dictionary provides coding instructions and target values for accurate data abstraction. The Full Specifications Data Dictionary is a more comprehensive version of the Coder's Data Dictionary, providing the technical specifications of the data elements and is available in both PDF and Excel format. The technical specifications include the following, usual and valid ranges, parent-child relationships, and precision, the use of values beyond decimal, just to name a few. Both data abstraction and quality personnel will use the User Guide document section. The Submission Population document provides who is included in the registry and the Inclusion Criteria document provides who meets the criteria for the registry. The NCDR has both procedure-based and process-based registries. Data Quality Report or DQR Companion Guide provides the process to submit data to the NCDR. The Dashboard User Companion Guide is an instructional guide designed to assist with becoming familiar with the dashboard features and functionality to support process and or quality improvement effort. The Executive Summary Measure Companion Guide provides information on measures and metrics specifically selected for each registry based on supporting evidence, guideline recommendations, or expert consensus. In the Quality Tools and Reference Documents section, you will find the following. Guidelines. These documents are developed using evidence-based criteria and are intended to assist healthcare providers in clinical decision-making by describing a range of generally acceptable approaches to the diagnosis, management, and prevention of a disease or disease process. The Appropriate Use Criteria document bridges the gaps in the guidelines and the indications outlined are derived from common presentations or anticipated uses, as well as from current clinical practice guidelines. The Self-Audit Tool document provides a means for sites to conduct an internal review of registry data to validate the data or identify problematic fields with the goal of improved data quality. The CMS Coverage Determination document is a determination of whether Medicare will pay for an item or service based on parameters set by the Centers for Medicaid and Medicare Services. The NCDR Risk Model Documents section provides several documents to enhance the understanding of risk-adjusted and risk-standardized measures. A risk-adjusted measure indicates the measure is based on a non-hierarchical risk model, which only includes patient-level risk factors. A risk-standardized measure indicates the measure is based on a hierarchical risk model, which includes both facility-level and patient-level risk factors. Risk model documents include NCDR Risk Model Introduction, NCDR Risk Model Part 1, What is a Risk Model?, NCDR Risk Model Part 2, Statistical Terms, NCDR Risk Model Part 3, How Risk Models are Reported in NCDR, and NCDR Risk Model Part 4, How to Interpret Risk Model Results. What is the difference between the Coders Data Dictionary and the Full Specifications Data Dictionary? The Full Specifications Data Dictionary is a more comprehensive version of the Coders Data Dictionary, providing technical specifications of the data elements. Why is it important to note the last modification date of a document? It is important to note the date of the last modification to ensure the downloaded document being used is the most current.
