This session will dig into the 2018 ACC/AHA Clinical Performance and Quality Measures for Cardiac Rehabilitation and data which demonstrate the individual and societal benefits of cardiac rehabilitation.  Participants will gain an understanding of the barriers to referral and strategies to increase referrals for Phase II cardiac rehabilitation within your organization.
