Join NCDR registry participants and CHD care team members in bridging the gap between in-patient and outpatient care of CHD patients. Best practices and guidelines for outpatient imaging and medical management follow-up for inpatient procedures will be discussed.

Examine a case with inpatient and outpatient care of a CHD patient.
Outline guidelines and best practices for the treatment of patients with CHD.
Identify, develop, and implement metrics used to track these guidelines and explore NCDR quality improvement resources and data available for analysis.
