Join NCDR registry participants and CHD care team members in bridging the gap between in-patient and outpatient care of CHD patients. Best practices and guidelines for outpatient imaging and medical management follow-up for inpatient procedures will be discussed.

Learning Objectives:

  • Examine a case with inpatient and outpatient care of a CHD patient.
  • Outline guidelines and best practices for the treatment of patients with CHD.
  • Identify, develop, and implement metrics used to track these guidelines and explore NCDR quality improvement resources and data available for analysis.
