Description:
Amyloid cardiomyopathy remains an under recognized cause of cardiomyopathy especially in older adults. Identification of patients with transthyretin cardiac amyloidosis requires an increased awareness of the symptoms, signs and diagnostic tools that are available for the discrimination of this progressive form of cardiomyopathy. Most patients are referred to our institution at advanced stages of the disease when they are less likely to benefit from available therapies for transthyretin cardiac amyloidosis.  We propose a multidisciplinary interactive electronic consultation platform as an extension of pre-existing infrastructure that we have developed to ease the patient referral process and reduce barriers to care for patients who require evaluation for cardiac amyloidosis in the Northern California region. Our project goal is to increase the number of patients who are referred to the University of California Davis for the evaluation, diagnosis and treatment of cardiac amyloidosis. We aim to develop partnership networks for the continued management of patients who have been evaluated and diagnosed through this platform.
