Description:
Moving beyond the "one patient, one provider" model of healthcare is needed to improve cardiovascular disease outcomes at the population level. This session will provide attendees with the knowledge and insight from a community and institution (VA) perspectives needed to implement care pathways that will help shift the current paradigm. 

Learning Objectives:

  • Recognize the concept of population health and discuss various metrics that are used to compare it across different communities.
  • Examine determinants of CVD at the population level, highlighting the importance of multilevel models that account for mediators and moderators of related metrics.
  • Evaluate innovative, data driven models currently being used to better understand population health needs and deliver targeted interventions to improve CVD outcomes.
