With one out of every five patients diagnosed with an acute myocardial infarction (AMI) readmitted to the hospital within 30-days of discharge, preventing avoidable readmissions is a national priority.

This Campaign is available to all Chest Pain - MI Registry™ participants.  The goal of the program is to reduce avoidable hospital readmissions for patients discharged with acute myocardial infarction (AMI) by supporting a culture of patient-centered care during the hospital stay and, in the weeks following discharge. These recordings are designed to support participants in that effort and will:

• Develop a team to implement the Campaign.
• Review their data outcomes and develop Campaign goals.
